Is your organization located in Dawson, Forsyth, Hall, Lumpkin or Union County?(Required)
Is your organization registered with the Georgia Secretary of State?(Required)
Has your organization distributed food to the public for at least 6 consecutive months?(Required)
Do you distribute more than 1x per month?(Required)
Do you serve more than 35 households per month?(Required)
Has your organization maintain records for the individuals to which you have distributed?(Required)
